Informed Consent

I agree that participation in It’s Yoga Kids, LLC is voluntary and at my/my child’s own risk and hereby absolve, release and hold harmless It's Yoga Kids, LLC, its agents, or officers, or Instructors from any and all liability for any injuries or damages that my child or I may suffer in connection with the activities sponsored by It's Yoga Kids, LLC or in which my child or I may participate.  

In case of an emergency and I can not be reached, I authorize the staff of It's Yoga Kids, LLC to obtain whatever medical treatment is deemed necessary for the welfare of my child. I further understand and agree that I will be financially responsible for all charges and fees incurred in the rendering of said emergency treatment. 

I agree that It's Yoga Kids, LLC may use such photographs of me and/or my child(ren) without my/their name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, website and social media.

With this purchase, I hereby acknowledge that I have read and I accept these terms and conditions.
